Granting: Grant applications
All grant applications must be received on or before January 30th. Each application
must include a narrative, no longer than seven pages, excluding the attachments
that address each of the following topics:

Organizational
Information, including
- A
brief history of the organization
- A
brief summary of the organization' mission and goals
- A description
of the organization's programs, activities, service statistics,
strengths and accomplishments
- The organization'
relationship with other organizations working to meet the same needs
or providing similar services, including an explanation of how the
organization differs from these others
- A summary of how the organization utilizes the services of other charitable organizations in achieving its goals
- The number
of board members, full-time paid staff, part-time paid staff and volunteers

Purpose of the grant, including:
- The situation, opportunity,
problem, issue, need and the Greater St. Louis community that this grant
application addresses
- The specific activities
for which the grant will be used and the individuals within the organization
who will carry out these activities, if known, as well as the qualifications
that such individuals are required to have
- The goals
of these activities and the specific ways in which these goals will be
achieved
- The time
frame within which the activities will take place
- How the proposed
activities or program will benefit the Greater St. Louis community
- Long-term
strategies (if applicable) for sustaining the proposed activities or programs

Evaluation
- Outline how
and when the organization will measure the effectiveness of the activities
or program for which the grant is sought
- Outline the
specific objective criteria for success and the results that the organization
expects to have achieved by the end of the funding period
- Outline how
the evaluation of the activities or program for which the grant is
sought will be used by the organization in the future

Attachments
- Financial statements for
the most recently completed 3 fiscal years (audited statements preferred)
- Project budget for which the grant is sought. If the grant
application is for general operating expenses, attach a budget for
the entire organization.
- A
listing of organizations, corporations and foundations from which the
organization has received financial assistance and the amounts of such
funding, including those from which the organization is currently soliciting
funding and whether the receipt of such funding is committed
- A listing
of board members and their affiliations
- A one paragraph
description of key staff, including qualifications relevant to this grant
application
- A copy of the organization'
(or its fiscal agent') current IRS determination letter indicating
its tax-exempt status.
